Online Forms

At New Garden Eye Care & Eyewear Gallery, we offer patient forms online so you can complete them in the convenience of your own home or office. Before your appointment, you will receive an email with a link to set up your patient portal where you will be able to fill out all neccessary forms before arriving for your appointment. If you can't fill out these forms ahead of time, don't worry about it! But please try to come a few minutes early so you can fill out the forms in the office. Thank you!​​​​​​​

​​​​​​​In the event that you fail to give us 24 hours notice of missing an appointment, a $100 holding fee will be required to reschedule. This $100 will be a credit on your account and may go towards any future bills or purchases.
